The Scottish international underwent surgery on September 8 to treat a minor and benign heart rhythm disorder, after the issue was detected through the use of wearable devices during matches and training sessions.
McTominay required minor surgical intervention and a period of complete rest before gradually beginning his return to the pitch.
Napoli's official social media accounts published photos from the team's training session at their training center, showing McTominay participating in drills with his teammates who are currently present, as most of the squad have left to join their national teams.
After completing some exercises in the gym, this marked the first time the former Manchester United player had participated in ball-based training on the pitch since undergoing surgery.
McTominay still needs to undergo examinations by Italian authorities and obtain medical clearance before being allowed to fully return to training and matches, given the strictness of Italian regulations regarding cardiac issues.
McTominay has been a key figure in Napoli's lineup since joining the team from Manchester United, and continued to shine for the club during the current season before the heart rhythm disorder was discovered.
